Pre-registration with the practice

You must attend in person to register with this practice and produce photographic identification.

Please bring your old medical card (if you have one) with you.


When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records. 

See below if you wish to print a copy of our health questionnaire.  You must bring the COMPLETED questionnaire to your new patient appointment.
